CHRONIC DISEASE MANAGEMENT PLAN 

You are able to  receive reimbursment of $52.95  from Medicare when you see a credentialled diabetes educator (CDE).To qualify for the reimbursment Karen requires  a Chronic Disease Management plan referral from your GP.

This may be called an Enhanced Primary Care (EPC)  plan by some

TYPE 1 DIABETES

Type 1 diabetes is a life long autoimmune condition. It is caused by the body's natural immune system mistakenly destroying the cells in the pancreas which make insulin.
