The diabetes educators at T1D Partnership provide an excellent standard of education and support with

  • Newly diagnosed diabetes

  • Type 1 diabetes

  • Type 2 diabetes

  • Pre-diabetes

  • Gestational Diabetes

  • MODY

  • LADA

 

They can assess your  needs and provide education and  instruction on all aspects of self care including but not restricted to ​

  • Blood glucose monitoring, meters and continuous glucose monitoring

  • Medication requirements

  • Exercise

  • Insulin start

  • Insulin pump commencement and follow up

  • Technical aspects of diabetes management

APPOINTMENTS

Your first appointment usually takes between 45 minutes to 60minutes,

This is to assess your needs and provide individualised information 

REBATES 

CHRONIC DISEASE MANAGEMENT PLAN (CDM)

ENHANCED PRIMARY CARE PLAN (EPC)

You may be able to  receive reimbursment of $52.95  from Medicare when you see a credentialled diabetes educator (CDE) or a Accredited Practising Dietitian (APD).

To qualify for the reimbursement a Chronic Disease Management plan this may be called an Enhanced Primary Care (EPC)  is required from your GP.  

The CDM /EPC plan is set up by your GP and allocates you up to 5 rebated appointments per calendar year.

These 5 appointments per year are shared between the allied health team you require to assist you with your diabetes management.

Allied Health professionals are:- 

  • Credentialled Diabetes Educators 

  • Podiatrist

  • Dietitian

  • Exercise physiologist

  • Physiotherapist

Karen or Jennifer can also see you  as a private client without an CDM /EPC or referral
