Your first appointment usually takes between 45 minutes to 60minutes,
This is to assess your needs and provide individualised information

REBATES
CHRONIC DISEASE MANAGEMENT PLAN (CDM)
ENHANCED PRIMARY CARE PLAN (EPC)
You may be able to receive reimbursment of $58.30 from Medicare when you see a credentialled diabetes educator (CDE).
To qualify for the reimbursement a Chronic Disease Management plan this may be called an Enhanced Primary Care (EPC) is required from your GP.
The CDM /EPC plan is set up by your GP and allocates you up to 5 rebated appointments per calendar year.
These 5 appointments per year are shared between the allied health team you require to assist you with your diabetes management.
